Text Messaging at Chrystal Clinic

This page describes how patients of Chrystal Clinic opt in to receive text messages, what those messages contain, and how to stop them at any time. It is published so that patients, and anyone reviewing our messaging practices, can see exactly what we ask and how consent is recorded.
Who receives text messages
Only established patients of Chrystal Clinic who have actively opted in. We do not purchase, rent, share, or accept phone numbers from any outside source. Consent is collected by Chrystal Clinic, for Chrystal Clinic, and is never shared or transferred to any third party.
What the messages contain
Messages relate to your care at the clinic: appointment reminders, scheduling, and follow-up about your treatment plan. They are sent individually by clinic staff. We do not send promotions, discounts, or advertising by text.
Message frequency varies, typically no more than 4 messages per month. Message and data rates may apply, depending on your mobile plan.
Where patients opt in
There are two places, both requiring an active choice. Neither is selected by default, and neither is required to become a patient, book an appointment, or receive care.
1. On the intake form
New patients complete an intake form through our practice management system at chrystalclinic.janeapp.com. The form includes a consent block headed "Text messages from Chrystal Clinic" containing the following language:
I consent to receive text messages from Chrystal Clinic about my care, including appointment reminders and follow-up about my treatment plan. Message frequency varies, typically no more than 4 per month. Message and data rates may apply. Reply STOP to opt out at any time, or tell us and we'll remove you. Consent is not required to receive care. See our Privacy Policy and Terms.
The patient then selects either "I agree" or "I disagree." Neither option is pre-selected. Patients who select "I disagree" complete intake normally, receive full care, and are contacted by phone or email instead.
2. After booking online
Patients who book an appointment through chrystalclinic.janeapp.com are shown an optional prompt headed "Never miss an upcoming appointment by enabling SMS Reminders." It contains an unchecked box labeled "Send me SMS reminders" and the following text:
Standard messaging & data rates may apply, messaging frequency can vary and you can update your preferences anytime.
The box is not checked by default. Patients may save without checking it, or close the prompt entirely, and their appointment is unaffected.
How to stop receiving messages
You can opt out at any time, by any of the following:
- Reply STOP to any message from us
- Reply to any message and tell us
- Change the setting yourself in your patient account at chrystalclinic.janeapp.com
- Email cc@chrystalclinic.com or call 1 (815) 200-1061
- Tell any member of clinic staff
Reply HELP to any message for assistance.
Your information
Mobile information will not be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties.
